The NHSN Biovigilance Team recently identified a defect in the Hemovigilance module that prevents users from submitting the Annual Facility Survey. On February 19, 2019 from 10am-1pm ET the module will be repaired to resolve the defect. The module will be unavailable for data entry during this time. While NHSN continues to fix this defect, please do not fill out the annual facility survey.
